REGISTRATION
Steps to register for WARRIOR Athletics:
- Fill out the Health Examination and Consent Form every year.
- Get a physical and have the Physical Form filled out by your physician. They are good for 2 years.
- Please fill out the Interim Questionnaire each year.
- Read and sign the Concussion Form each year.
- Read the Athletic Handbook and sign the last page each year.
- Submit all of these forms and the Athletic Fee (please see below) to the Middle School/High School Office.
