High School Athletic Documents

• IHSAA Health Examination & Consent Form (New physicals are required at the start of your 9th & 11th grade year.)
• IHSAA Interim Questionnaire (10th & 12th grade year.)
• Concussion Form (All four years of High School.)

• Watersprings Athletic Waiver (All four years of High School.)

   

Middle School Athletic Documents

• IHSAA Health Examination & Consent Form  (Physicals are good for two years.)
• Concussion Form (All three years of Middle School.)
• Watersprings Athletic Waiver (All three years of Middle School.)

Athletic Fees 

Athletic fees are assessed to help offset the costs of uniforms, equipment, transportation and facilities. Fees are due prior to the start of the sport season.

Middle School Sports:

• Football              $150
• All other sports $100

High School Sports:

• Football              $200
• All other sports $150
